Physical Examination

Students registering for Athletics must have a current physical examination form downloaded for the School Nurse and or Athletic Trainer’s review before the student is authorized to participate in practice or tryouts. Physicals are valid for 1 year from the date of the exam. Download the physical form by clicking the Athletic Team Registration icon, complete a family account, and follow the download instructions.
